Health Forms
All health forms are submitted through the FACTS Family Portal during the enrollment/re-enrollment process. All students are required to have a CH-205 with an up to date immunization record.
Students with allergies or other medical conditions requiring medication at school must have a Medication Administration Form on file with the school nurse.
